People who are so deep in their delusions see their delusion as the truth. Deeker does show a smattering of understanding of the outside world - he knows that some people find his fetish 'odd', but that's about the limit. I think we should all be thankful for one thing, that this thing will never breed and have children. If he had kids, this is one of the few things where I would advocate sending the cops over to someone's house. With everything he is presenting the public, the idea of him having a child under his care (())__(()). That poor child will have very little chance at adapting to a normal life.
