Nothing, but the visits are court mandated as part of the custody agreement they arrived at. Which means Faith has to provide medical documentation that Xander was actually sick/reschedule the visit. If it happens repeatedly or if the visits aren't rescheduled, then Ralph is within his rights to seek legal action. He's already milking the "THEY WON'T LET ME SEE MUH KEEEEEEEEDZ!" angle with his women-hating audience, it'd be really stupid of Faith to give him any more ammunition.
Likewise, if Ralph shows up drunk or on drugs, Faith can also take legal measures for her and Xander's safety.
