They're shipping in the raw estriadol, which is not prepared for human consumption like Estradol is, and not regulated by the FDA.

I'm still not sure why HRT is really necessary beyond just being part of the "culture". The biggest problem I see, besides sterilizing everything, is impatience. One of the keys to being good at certain jobs, is patience and knowing the process must be adhered to. Most trannies are obviously impulsive and demand satisfaction on a whim; I can't make a prediction of how it will turn out, but I know there will be parts of instructions that aren't followed properly, because someone's feeling a little too dysphoric and needs it right now.
