How do I write a vaccine booster religious/philosophical exemption request

Get a letter from your attorney about how god created the Nuremberg code and will come down from heaven to ravage your employers asshole if he tries to jab you.
 
It helps to maybe ask your pastor or rabbi or someone from wherever you practice your religion to write you a statement and use it as proof for a religious exemption. Having a written document from someone who is an authority on those matters might make your statement stronger.

Oh I don't actually have a religious reason, but the form allows a philosophical or religious one

Tell them you are part of a religion (Christian/Jew/Hindu/etc) and say that you have a "sincerely held religious belief" that forbids you from getting the vax. Just stick to that, say no details besides this. The fewer details you divulge the harder it is for them to poke holes in it.

If that fails, then find a sympathetic priest/pastor/rabbi/imam/whatever, to write a statement vouching for you. Or if that fails, find a sympathetic doctor to write a medical exemption.

my university fortunately isn't as cucked as Stanford, look at this shit. mine is just a paragraph to fill in text. i'm deciding whether I should go the religious route or ethical/moral objection route

What did you do for your exemption request for the first vax?
I did the first vaccine a long time ago after being convinced by my family. But now they are mandating even more and I've decided endless vaccine mandates is not right

Good for you for getting off the vax train now.
You should think more seriously about a religious exemption, as they are harder to deny than a personal belief one, and many institutions are reluctant to press people too hard on them for fear a of a lawsuit.
The Stanford form you attached is a very high bar for a request to meet, and most places are not as strict, but I am confident that you will be able to come up with something that will reach even that high bar provided you have a sympathetic religious authority/clergy member.
