Yes. I'm sure they were both born defective to begin with, but they're both long-term drug users (I'm including alcohol as a "drug" in this case, too) and though plenty of users and abusers (especially stoners) will swear otherwise, long-term drug use causes permanent physiological damage to the body, and in particular the nervous system and brain. Organ failure is just an added bonus.
Long-term alcohol abuse has been shown to "cake" the links between neurons in the brain with this weird garbage that disrupts the signals (which never goes away and cannot be removed clinically), leading to diminished recall, slower response times, confusion and disorientation, and other symptoms associated with early-stage dementia.
Weed just sort of slows everything down over time (it takes a long time and a lot of consumption, but it does happen). The harder drugs can do anything from scrambling/rewiring the brain (LSD) to just destroying grey matter (cocaine, heroin, etc.) which wreaks havoc on normal brain function.
None of these effects are reversible. Damaged neurons and links never heal. Grey matter destroyed by drug abuse is never regenerated (the brain does continually grow new cells, but not nearly at the scale required to replace large swaths of damaged or lost cells). This is why strokes are so dangerous and destructive -- the damage they cause isn't something the body can repair on its own and we don't know how to do it clinically.
That damage adds up over time and it always results in deterioration of cognitive function. A big reason junkies are often dangerous (and rarely able to be fully, permanently rehabilitated) is they are quite literally destroying their brains with their drug use, making them dumber and dumber over time as they continue to be controlled by their addictions. A "fresh" drug user can still think straight and reject dangerous or violent actions and hare-brained "schemes" to get the next score. Someone who's been scarfing down opiates for a decade is practically operating at the level of a spider monkey that can talk.
TL;DR: They're both nuts, but drugs and alcohol have made them both absurdly stupid and unable to control themselves. Jewel is late-stage broken; as you noted, she absolutely has no self-control and acts exclusively on impulse. She lacks the mental acuity to predict outcomes or plan ahead. I'd be surprised if she could plan a day's activities in advance and actually get it all done without outside help. She fights with police because she is angry. There are no other considerations. She has no comprehension that they will punish her for fighting with them. The thought simply never enters her mind. She's going to end up institutionalized eventually.
Goocheese is well on her way there. She's still somewhat functional, but as the airport video demonstrates, a fair portion of her higher cognitive functioning has already packed its bags. She didn't strike me as super drunk or high as a kite in that footage. Clearly she's on something slowing her down and stopping her from focusing, but there's something deeper at work there too. She's not just being stubborn about boarding the flight. She's not even processing what the guy is saying. She's not choosing to ignore him; it's as if she doesn't hear it at all because her brain is just dropping it on the floor. Look at how many times he has to tell her she's "making people nervous" before she even acknowledges it and starts repeating it as a question.
It's gonna keep getting worse, and it's going to be really funny to watch (from a safe distance). God help their fellow travelers and any poor shmuck who has to deal with either of them though. I wouldn't wish that on an enemy.

I watched some of Jewel and Attilas car stream. She is not pregnant.
