Possibly but unlikely, he's got neck veins left to shoot in that look relatively clean. When circulation starts to get bad from getting jammed up with drugs/infection those start to go. Also scratching and lack of sleep and proper food will further the process. He needs help soon.

If the discoloration isn't just dirt and grime which the swelling leads me to think is the case, this guys been a junky who had leveled out for a few years but inside the last 6-months to a year really hit some lows and got into the heavy usage of some low quality shit (think shooting gallery grade junk) he could be dead inside anywhere from tonight to 5 years from now with sporadic ER trips.
