Certainly within the realm of possibility. Not exactly typical but not impossible either. IIRC HIV is "happiest" in blood and relatively fragile. Sensitive/readily passed through flesh like genitals or mucus membranes aren't going to be perfect at stopping it. If they have sores from other conditions then they might as well have been in the fuck room at a bathhouse.

Ralph is most likely to get HIV thanks to sharing a crusty coke straw with his busted nose full of open sores and snot. Of course his love of bungholes and hookers could get him too.

I believe the only way a man can get HIV from oral sex is if they have open sores, like if Ralph had broken skin and the e-thot had open sores around their mouth it can be transferred that way. It's more common for a man to transfer it from their semen. So if Ralph was carrying HIV he could pass it onto anyone who he's fucking and orally raping a person.
