Regarding Yaniv's psyche, I think that he doesn't have a very well-developed theory of mind. Basically, when children grow up, they will realize that people don't necessarily think the same way as they do. People with a developed theory of mind are able to guess what other people might be thinking intuitively. Generally, most children develop it by age seven, but those with autism or physical disabilities hindering communicative input (ie: deafness) may not have a well-developed theory of mind. Those people tend to be terrible liars and may possibly be less considerate of other's feelings (albeit unintentionally). This makes me wonder if Yaniv is actually not capable of understanding how badly he hurts other people, and why he compares himself to actual victims of assault.

NPD does get diagnosed, it's just usually not the presenting problem. They show up to be treated for depression, anxiety, to get stimulants for "ADHD" so they can work on their Important Plans, or to talk about how traumatized they are by the idiots who just don't understand how right they are. A good diagnostician who talks to them for more than a couple sessions will cotton to the Axis II underlying it all and start carefully trying to tease that out while humoring them sot hey don't flee.
