I think he's on HRT for a few reasons.
While it is possible that Jon is on hormones, I doubt that any credible doctor would intentionally prescribe them for safety and legal reasons. Jon has already been shown to be mentally unstable. He is also not in the best of shape, making HRT an option out of the question under the legal route. If Jon is in fact taking hormones, he is most likely swiping them from Miriam, who might have medication for menopause.
