I'm sure heroin will screw with your body in a myriad of ways, but I've never known it to cause infertility in a person. I think transsexuality is worse, provided you go through with transitioning fully. As the effects of transitioning aren't easily reversible—especially if you cut your penis off. No amount of testosterone re-introduction or implant removal will ever change that.
