I wouldn’t be surprised if the extended family are not fully aware of Kevin.

Yes. This exactly. Phil's grandparent(s) are in their dotage and it would not surprise me at all that Phillip lied to his grandparents about Kevin---just don't get to close and don't talk too much and they think you're a female and this is what the greater family would be told if they checked in. I really do expect that Penny was not as honest about the situation as he would like others to think. Pure speculation here, but he might also be very good at keeping the other family members at a distance, as in there are some family politics and dynamics we have no idea about, but Penny knows how to exploit this to his advantage.
Perhaps, again pure speculation so take nothing of this as evidence, the caring for the grandmother with her dementia is a white elephant of sorts.
The bulk of the family, in this scenario, may want to put both parents in an assisted living situation so they can be together and the burden of care is removed from the father, who is as we have seen, a very old man.

Penny could move in with his "girlfriends" who are both "experienced carers" and as a result the implication is that Penny works as a handyman and maintains the properties, while Kevin and Jen are supposed to be caring for the parents.

If the extended family have no reason to doubt that, I.E. no phone calls from parents screaming about strange mannish women masturbating with enormous dildos and eating shit in front of them, then perhaps they assume all is well and let sleeping dogs lie.

Of course, Penny may even have told the extended family that the parents are both getting delusional and might start calling family members and up claiming such things.

Phil and Kevin know this is the end of the line (grift) and there is nothing after this but a tent under a bridge, so, there's plenty of motive for deceit, mostly to stay in comfortable digs, and the TUR has demonstrated adequate capability in deceit (and possibly fraud).

Given Kevin's character and public behavior, does anyone think a transgender piss fetishist should be adjacent or near the elderly, children or the infirm?

Maybe Penny’s grandparents have always been ok with Penny having odd degenerate friends lurking around, and actually appreciate the company.

It’s unlikely that this is the case, but unless distress is clearly indicated, then there is nothing a bunch of strangers on the internet should do.

Probably keeping themselves willfully ignorant. Dementia and elderly care is a heartbreaking thing, I bet the family are relived as fuck they are being "cared for" by one of them, in the home, and that it's not them left holding the can.
