If he's gone back to living off of cheap, processed food (so, in his case probably chips, cheese, and cocktail pep level meats) and drinking soda for the bulk of what he eats outside of the free shelter meals, he's probably got a combo of weight gain, water retention, and the 'beetus coming back.

The only reason he was allegedly able to go off of his insulin as a type 2, I'd guess, is because he lost a fuckton of weight and when he could buy his own food was choosing things that weren't always super salty or heavy carbs.

Now he's at the mercy of what the shelter(s) serve and between those one or two meals per day he's got to stick to food that he doesn't need to refrigerate and that he can eat before he hits the shelter again in the evening, and most of that is high salt, high sugar, high carb stuff which isn't good for people who aren't diabetic to have for the bulk of their diet and it's CERTAINLY not good for a diabetic to eat like that.
