I don't know if it was just Alabama\Walmart\the shitty gas stations I worked at, but I never needed a note and just didn't get paid. Now in the Portland metro area, the state mandates that you accrue sick time and you don't need a doctors note unless you're doing fmla stuff.
So, no, I've never heard of needing a doctor note for one or two days of sick. 3 or more? Yeah that's the FMLA territory.

My background is mostly fab shops in the south, but generally the smaller ones don’t care as long you’re not out for 3 days or more in a row, 2 days or less a foreman usually accepts a verbal “hey I had the flu/cold/infection” whatever. The only places I’ve noticed that care are the bigger/corporate shops. In those places, people can abuse the good will and take out multiple “sick” days a month.
