I believe the Unique User ID system that is now used has a history of all your account names. Beware joining if one of your past names link to somewhere else.

You probably do want to get one of the servers that allows for moderation tools and maybe anti-griefing.
Since you want to stick to vanilla, at least getting a server side only mapping mod that people can see through the web would be nice. I think JourneyMap can do that, but not 100% if it can function like that.

Yes. Username changes are set up so that you can get a username that you prefer, and are not designed to circumvent bans or anything of that sort. Previous usernames are recorded and visible to anyone.

AFAIK, account email is never shown to anyone, moreover you can change it if you remember security questions (if you don't, you have to bug support with proof of purchase, and wait about a month for answer). The real problem, as stated before, is sites like namemc.com, which show full nickname history to everyone, so even if you change it to something neutral, anyone can still view the older one. Best bet is to buy someone else's abandoned account from a shady Russian site for about 5$, and then play with it, changing nickname to something more fitting if needed.
