That's pretty much all of us. He's been rotting for over a decade now. He'll probably get some amount of inheritance and live off of Neetbux for the rest of us life once his father dies. There's probably a larger number of people like this than we think. Completely NEET-ed away from the rest of society, living off of whatever scraps the government gives them for their "disabilities", surviving kinda around the poverty line but with enough cheap entertainment to keep them from blowing their brains out.

Honestly he looks like what Cosmo would probably look like normally aged up 10+ years to me. It's not like he was the pinnacle of health before...he's a speedrunner.

Dude what life. He spent 10+ years doing nothing and interacting with no one. He was doing badly before that too. Bummed out of art school, no actual employment. The speedrun thing was a 15 minutes moment. Even John numbers is a bit of weirdo these days. No one won that contest as far as I'm concerned.
At 37 with nothing to your name and with the knowledge you destroyed yourself both physically and mentally by drugs, hormones and involution, this guy is aware he has nowhere to go.
