I doubt it. When you become a recluse, you tend to either get rail thin because you're too sickly/moody to eat or balloon up because you eat out of boredom/for comfort. He's probably less starving-skinny and more out of shape dyel skinnyfat beneath his clothing from a diet of almost exclusively soylent.

In full body shots it doesn't look like he has the stomach pudge you get from sitting on your ass all day but also eating somewhat normally. He looks like a permanently worn-out skeleton with straw hair. Besides, Narci would have to be eating hardly anything to not gain weight on hormones, even dodgy Chinese hormones.

The problem is that he was originally pretty thin and as he 'transitioned' and started replacing meals with an easier option for what he was doing, it has broken down his body to nothing. It would be classed as an eating disorder, he is not anorexic though as of this moment. Its highly likely his meals are literally just bottles of soy and a bit of trash. Whatever the case, he is consuming at the point between maintaining his body weight and at a deficit.

I get having Soylent as a once in a blue moon meal replacement if you're hard pressed for time, but Cosmo for about a year really did buy in to the marketing of it being a total diet in it self. Cosmo did complain about food being a hassle and Soylent being a solution for that struggle. From what I understand now Cosmo has somewhat weaned off from only drinking Soylent starting early last year (https://twitter.com/narcissawright/status/824336882142044160) but the damage it left definitely still shows.

Narci takes actual hormones, I don't think he ever bought into the meme of plant phytoestrogens being the same thing as human estrogen.

I also didn't know he lived on a diet of mostly Soylent tbh. What did his stomach and intenstines do in a previous life to deserve such a punishment in this one?
 
I dont know if this is recent but the past couple of streams had narcissa talking to chat, she rocks back and forth and makes a sort of hissing noise its really odd, might be all the weed they been smoking/edibles eaten, on that note, "nick12209" and a couple others were egging narcissa on to take another hit to which this person "killdeerstudios" was opposed to, convo and turn out was as follows:
