How do you "realize" you are transgender when you are like 40 years old?

I am progressive enough KF poster to believe that a very small portion of people are trans and that its a mental illness to be treated. But the ENTIRE point of it is that you are fucked up enough that you cannot feel comfortable in your own body and need the treatment. That should be cemented in your personality by early adulthood at the very latest.
