Yeah, it's controversial because we still know so little about the mind in general even with stuff like depression. Jamais' explanation was pretty good for a quick summary of the types you'll find posted on here. The main thing is that in "legitimate" cases where there's not physical brain damage or at least every one that doctors have essentially said "this is as close as we'll get" the person's fragments (to use a lack of better term) are generally all versions of the same person. They may be a more rage or vengeance focused version, etc. And usually the person remembers the gist of when they disassociate. It's not like they disappear. They just may forget details like anyone can do.

There's been basically no "real" cases found where the person "disappears" and suddenly a trans succubus fronts for hours on end. Or whatever other anime or video games or whatever they like. I believe the closest cases have been where the person fragments into an identity like this as a defense mechanism but it's not a different person, it's still a version of themselves like they see themselves as a strong manly version or whatever. The doctors remain skeptical but it at least fits within the wheelhouse to where they can treat it and since that's their goal they don't really quibble about it.

You can find YouTube videos of legit DID types being interviewed or whatever during incidents and they're mostly just confused when they disassociate, and the fragment generally reacts to that feeling, if the doctor can get to know the fragments you'll see the person calm down quickly because the doctor they know is there and oh, this isn't a bad situation. As the fragment calms down they usually drift back to their "normal" state with their main thing being confusion.

Also as noted, this dude is so transparent anyone would figure out why he's seeing a therapist in the first place. Many therapists/psychologists/psychiatrists/etc. aren't going to debunk your horseshit the first time they meet you, and it's not a greedy doctor thing it's a "we have to get on the same page" thing. So they'd ask him to describe this or that. And they'd have all kinds of preplanned questions that will definitely reveal what this dude is up to.

The real difficulty for a doctor is not dealing with this dude if he has actual DID, it's the fact that the dude clearly doesn't want to be cured in any case and wants to LARP as a MINDSPLINTER HERO so the goal, if anything, would be to basically trick him to act normal enough for a job or whatever and then he can play online or on his own time. Obviously the therapist would have a bunch of big fancy words and big fancy theories behind this that I don't.

I assume he's seeing a therapist as part of collecting welfare of some kind. He lives in Portland and doesn't exactly seem very homeless, that "getting ready for pride" image has a bookshelf full of boxed games behind him.

It sounds like you're describing his "layman gamer" channel. I've only seen a few episodes, but I think that show is meant to be opinions by his brother who doesn't follow gaming news and just likes what he likes. I don't know if it's a parody.
