I remember this one, G4 had a review for it strangely enough.I used to play this platformer years ago called karoshi. The object was to kill yourself. I don't know if it's really obscure.
I finally played this because it was free on Steam for a minute. I thought it was an "okay" Hotline Miami clone. My issue with it was the excessive dialogue and it gives you the option to stealth it out but you could totally run 'n gun the whole time. It might take a few extra tries (and I was playing on the hardest difficulty) but I had more fun just going "Fuck it!" Not a lot of fun weapons to use either but I appreciate the attempt that was made to differentiate it from Hotline Miami.Intravenous, an Splinter Cell and Hotline Miami mix. That will probably fail because it was no publicity and the dev have some back bone in twitter.

Intravenous on Steam
Teeth-clenching stealth, followed by tactical hardcore gunplay for those not looking for a quiet way in. Intravenous redefines challenging stealth-action and will keep you on the edge of your seat!store.steampowered.com

Interview with developer:
How much of the game is there? Looks really cool.Agartha. A cancelled Dreamcast game where you play a guy wandering around a snowy Romanian village during a horrific pandemic caused by contaminated water. There's some massive underground city beneath the village that is the cause of everything. It's a survival horror. Or it would have been. You can download a version that's cobbled together from a corrupted demo disc and see what might have been. It's a shame that Sega gave up on the Dreamcast and this project never resurfaced on a different console.
