Shad was able to hide his own pedophilia by using his shocking edgelord persona as basically a troll shield. Whenever someone would call him out on the sick shit he would draw the default response from his friends and fans was "Well yeah that's what he does he draws extreme shit because he's edgy" they sort of wrote it off as having no deeper meaning. The men are told to wait outside while the birth is going on. The wife’s cries of pain tend to cause a lot of men to either pass out, panic, or get aggressive. So it’s easier to just send all the men out in the waiting room while the birth is happening. It’s the same logic as “go boil some water”.
