Kind of late here, but let's not forget the Mu Training from Earthbound (sorry, but Giygas is just WAY too overused of an example):

And out of all of Undertale's Amalgamates, Snowy and Reaper Bird were easily the ones that stood out the most:

if you've played LSD dream emulator you may know that eventually textures will change to other things. i wasn't aware of this the first time, and i didn't know what the fuck is going on. then the grey man started showing up, which i also didn't expect.

what, do you have HIV or something? the labs weren't all that freaky. i recall actually getting annoyed fighting those things, but the entire time i played through undertale i was drunk as shit so what do i know

Most of MOTHER 3 contains one WTF moment after another. I've left some of the highlights in the spoiler below.

  • The reveal of the Ultimate Chimera. Running away from it also counts.
  • Miracle Fassad.
  • The O2 machines.
  • While they are implicitly crossdressers, my first time seeing the Magypsies counts as one of these.
  • Hinawa's death.
  • Porky's reveal.

the first mutant reveal with punkert. The human Toilet. The Dulien grunt song
 
Every dive sequence in Observer is a new exploration in surreal horror. Justifiedly so, because you're rooting around in another person's unconscious mind for information, but still.
 
The second encounter with Scarecrow in Arkham Asylum, you're just walking down the corridor and you're just trying to get to progress the story. Then, the game just stops and it looks like the game is glitching. I legitimately thought there was something wrong with my PS3 disk.

The opening cutscene plays and I'm extremely annoyed. I'm playing as the Joker as he's switched places with Batman. Then it switches to Batman's point of view, Joker shoots BMan in the face. Game Over screen and the hint message is fucky, it tells me to move the middle joystick to avoid getting shot in the face.

Then Batman digs his way out of a grave and navigates some nightmarish hellscape, avoiding the gaze of a gigantic scarecrow. After he was dealt with, you fight some skeletons, just skeleton after skeleton. Some are bigger than the others...then you're back in Arkham Asylum surrounded by unconscious enemies.

That was a wtf moment that stuck out to me.

What about near the end of the game where Andonuts is operating on Ness & co and putting their soul(?) or internal organs(?) into robot bodies? That scene had Ness, Paula, Jeff and Poo were seen lying down whilst there were sounds of power tools and hammers heard in the background. Surely that would be considered a WTF moment in Earthbound too?

Xenogears has plenty:

  • The Soylent System
  • The reveal that humans are spare parts to revive an ancient interplanetary weapon
  • The main characters get reincarnated every few thousand years and Fei gets to see Elly die every time
  • Almost everyone (including Citans wife), minus the main characters is dead by the end of the game and there’s only one settlement of humans left.
  • Citans General asshattery. Especially when he knows the Solaris food is made out of recycled humans but doesn’t tell Elly and Fei that as he watches them eat it.
