The difference between them and Ralph is that they had at least one person in their lives who told them the truth about themselves. Ralph has us but lacks the humility to look at himself honestly. It’s possible he could recover, but it’s not probable to put it mildly.

That’s really the core problem. Anyone who has or would tell Ralph the truth or even offer help has become targets of his attacks. Yes, Jim showed infuriating restraint there but I understand why he did it. He was giving Ralph an opportunity to back down. It was predictable that Ralph would waste it and consequently chimp out at Jim for telling him the truth, but it was still a decent thing for Jim to do. His side of the street is clean.

The fourth step is hard for most folks in recovery as it’s a crucial tool for self reflection and setting the groundwork for a new way to live.

Addicts might get sober, but making "good lives" in an earning sense requires having the inherent ability and interest in making one. And that part requires humility as well, because if you've fucked up enough, whatever you did before may now be unattainable due to reputational, work history, and intellectual damage. In Ethan's case, he has never done anything of value, so there is not even anything to go back to, even in a reduced capacity. His "career" is being an alcoholic pig, and he has no other skills.

Similarly, having "beautiful and functional families" requires wanting one and having the innate ability and interest in being part of creating that. At no point has Ralph had either one of those qualities. He is a pig, and he picks (let me be nice) questionable partners who accept his abuse and whom he doesn't respect. He respects no one, and that didn't start just because he got rando approval for it. He's just a bad, hollow person of limited intelligence and zero quality, and that exists before and during any addiction, and 100% would exist after any hypothetical sobriety (which he won't ever be interested in achieving, anyway).
