I believe it's now just a catchall used so someone can just say, "oh no guys my [fake mental illness] is being triggered by [loud noise, tumblr meme, person disagreeing with them] and now I'm dissociating please donate to help."

I've always assumed they mean they are dissociating from reality itself, as in they no longer connect to reality or consider it real. But it's always the dumbest things that do it.
