Because they're incapable of relating to a character in a healthy way.

Back in the day, people would appreciate and relate to characters as-is. Sometimes, they would gender-swap those characters, but it was more frequent they'd just enjoy the character itself. That's because people knew you could relate to a character without being that character. You could cosplay as the character, or roleplay as them, or even kin them, but it stopped there. It stopped at a healthy place. Going further would get you rightfully shamed or bullied.

Now that "cringe culture is dead," those limits are lifted. That's why you see people pretending to have dissociative identity disorder - because it allows them to roleplay and cosplay further. It gives them a shield to hide behind. It became a sort of kin-olympics. Sure, you relate to this character, but I am this character, so you need to leave them alone. "You're cringe" was twisted and rotted over the years into "you're ableist/transphobic/racist."

Top scars on a character symbolize "Hey, this character grew up as a girl! This character once had tits! See these scars? That means those tits got taken off. That means he also has a cunt, like me! He takes it the same way I do! I feel so validated that this male character was actually a girl, just like me!" They're basically fetishizing themselves in a really, really pathetic way. That's why you rarely ever see this sort of art coming from a mentally healthy individual.

I love how TRAs will say mastectomy scars are tiny and barely visible (to encourage women to chop off their boobs) but tranny artists will make them THE BIGGEST MOST DEFINING FEATURE OF A CHARACTER. Total cognitive dissonance.

troon males spend too much time cooming or excreting feces from their rotpockets to actually do something semi-useful like drawing or writing fanfic, so you don't see MTFs in fic/art unless an especially try-hard handmaiden decides to try their luck in getting extra clout.

also, even FTMs realise that tranny males can never live up to the fantasy, no matter how much lip service they pay. imagining a girl as a boy when you're a girl yourself is probably fun because you don't really have to think about any of the stuff that comes with being a man that they perceive as gross, like the body and facial hair and the dick bulge and whatever, but those traits are inherent to MTFs for obvious reasons, and no makeup or girly outfits will hide that.

The "context" is that Luz is consistently tomboyish in appearance, and the creator of that image is among those who are really, really weird about people being more than mildly askew of conventional gender roles.
