It depends on the condition. The vast majority of intersex conditions are caused by something other than faulty sex chromosomes, and the ones that do usually don't affect the genitalia to such an extreme (ex having small testes as opposed to ambiguous genitalia) and usually comes with mental and emotional disabilities. (Trisomy X have IQ ~ 20 points lower than average, XXYY may have ASD, ADHD, and mood swings, Kleinfelters may be delayed and poorly coordinated, etc).

The others are other genetic abnormalities, overproduction of this, insensitivity to that, or being exposed to something in-utero.

They probably get it mixed up because of cases where a male character was magically turned into a woman but still has a penis etc, not paying attention to the difference between that and actual gender transitioning.

I am all for a good reinterpretation of characters which is commonly practiced in the comic book industry, but the redesign has to look like the character in question, not a moldy potato covered in pox marks which leaves the viewer staring absentmindedly at the page questioning the thresholds of artistic liberty.

The version of the one with freckles, or mange or whatever the hell is on his skin is the person's newer reinterpretation of them, compared to the other one.
