The systems didn't "fail" that day, they simply found a vulnerability to bypass it completely. Shit happens, we are constantly working to improve. Systems aren't perfect from the go. And to this day, some 'p still slips by, which is why media approval is there.
If you look at any automated sorting system in real life. There's always a bulk filter done by a computer to lower the load on the last human filter. Same goes here. 'p detection and anti ban evasions do the most filtering, while staff only has to do media approval and old school ban evasion detection. Which is how we improve the system, the team is constantly watching out for ban evaders and writing down how they evaded the bans to patch every new method. Its a slow but effective process, which is how we got to today's 'p free sharty. 99% of offending images are already discarded by the system, so the media approval queue stays fast and efficient. New users barely experience any delay since media approval is only used on the newest users. Objectively the site is currently in its best state (regarding the 'p spam, other aspects like content quality are subjective. However the amount of 'p on the catty has been ZERO for several months, objective statistic no amount of seething can deny)
