Surgery on May 1st. Pic is 5 days post op. I did keep the bones. Intense pain afterwards, slowly getting more manageable. Dr Leif Rogers performed the surgery.

I also got a scapula reduction and rib remodeling at the same time, but I don't think you can really see the scapula? and the Ribs are pretty subtle, I think I need to lose more weight and/or get some lipo before the results of that will be particularly visible.
At my pre-op they measured my shoulders edge to edge at 18.5 inches and then 17.3 at my post op. Overall I think the results are positive but not amazing, I had already broken my left clavicle over a year ago and decided I wanted to get it fixed and if I was gonna do that...why not get the feminizing version!!

I had bottom surgery at Dartmouth Hitchcock Medical Center on March 22nd, 2024 with Dr. Nigriny in Plastics and Dr. Moses in Urology. The two worked together to perform a full depth penile-inversion vaginoplasty with a scrotal graft. I had a small revision with Dr. Nigriny on December 16th, 2024 to address an over-sized and over-exposed clitoris as well as som extra skin in my labia majora.
My vagina works pretty well for the most part. I can orgasm solo and with a partner, and I have about 5 1/2" of depth using the largest orange dilator. When I am aroused, my labia minora swell in a way that is really affirming, not to mention it feels incredible. Even though I don't technically get wet, my vagina usually has some moisture to it and a little spit is enough to get some fingers in and have a good time. My crotch looks great and pasable in tight shorts, and yeah the whole thing is just great and I am so glad I had bottom surgery. In short, I love my new vagina. I do, however, have some critiques of my results as well as my experience with Dr. Nigriny that I want to share with whomever might find it helpful. I'll start with my results:
Overall, I find my vagina a little too forward-facing, like I wish the whole thing was tucked under my a bit more. You may also notice that my clitoris is still pretty frontal and exposed, and I don't have much of a hood over it. I don't love this aesthetically, and from a functional standpoint I wish I had more of a hood so that I had other options besides direct stimulation to my clitoris. I also think it looks kinda weird. But hey, my orgasms are absolutely incredible so I wouldn't want to try and change the position of my clit and risk losing sensation. There is also some pretty visible scarring along the midline above my vulva. It seems like the midline incision made to create my hood stretched during the healing process, both during the primary surgery as well as when it was redone for the revision. The result is a hairless patch of discolored skin that extends up into my pubic hair, further accentuating the forward-facing nature of my vulva. One other major critique I have (that I may address in a future revision someday) is that my pee stream is a bit of a spray and also shoots forward such that I have to focus on keeping my stream in the toilet when I sit down to pee. Peeing outside is also very difficult, and I usually end up with pee on my shoes and left thigh.
Regarding my experience with Dr. Nigriny, I had a really hard time interracting with him. I have detailed this in previous posts, but I found his bedside manner to be pretty terrible, and I never really felt like he was listening to me, moreso that I was just a nameless faceless patient in his office. I tried to give him feedback about the forward-facing nature of my results, and it fell on deaf ears. Dr. Moses from Urology is very nice and personable, and so together I think they make a good team. I have seen Dr. Nigriny wearing a big American flag pin at recent visits, which causes me to wonder if he is not politically aligned with trans rights, but this is speculation. Most of the time I felt like a client off of which he was making money, rather than and individual with wishes and hopes for her vagina. If you seek top-notch aesthetics and good bedside manner, maybe look for another doctor.
You may be asking, why did I go to Dartmouth, why did I choose to do a revision there after an initial experience that left much to be desired? Well, my top priorities when seeking out a surgeon were: proximity to home, technical proficieny, being cared for by a renowned medical facility, and ease of dealing with insurance. Dartmouth easily met all these criteria. I live in Central VT, so this hospital was only an hour an change away from me, which was very handy for follow up visits. I also chose to stick with the same team for my revision because I felt there was a higher chance of retaining sensation (given that Dr. Nigriny knew what was under the surface, so to speak). If I ever to another revision, I'll seek out another surgeon and hopefully address some aesthetic concerns as well as my pee stream and lack of a clitoral hood. If you know any doctors that do revisions of other doctors' work, drop the name in the comments!

Hey all, got surgery almost two weeks ago and the care back home is going well. I’ve basically been doing everything by the book and have only had minimal issues with dilating.
My one main issue is that I don’t think I’m allowing enough air drying time, so I want to make sure that this looks normal and is healing okay?
I have relatively low pain, mostly coming from the very front in occasional sharp bursts but nothing that has me laid up in bed all day. My swelling is low enough now that I can comfortably put my legs together standing up.
Any insight would be greatly appreciated, please be brutally honest.
An update from the lass with both a hotdog and a hallway: at the cost of her doofy looking pool-noodle, she went to the ER and had to have round-the-clock IV antibiotics because, as it turns out, she had an allergy to the stitches used for her skin graft! Ive had issue after issue with my forearm graft site. Had an infection under the skin. Not under the graft site but under the skin attached to the graft site. I was given 5 days of Keflex and it seemed to clear up.
A week later i wake up one morning with my arm swollen and red. Again! I sent pictures of my arm but i think because of my tattoos they didnt see how swollen and red my arm was. So i listened to my gut and went into the er.
Ok to im sweaten it. Im worried im being a worrier. And theyd probably toss me a prescription of antibiotics and send me home. I went in not at all prepared! With in ten minutes im being pulled in and put on a bed. Im told im going to be admitted into the hospital over night! Im like Oh shit! Long story short i was in the burn unit for two nights. And given round the clock antibiotics through IV. I cant remember when but at some point they took a bunch of swab cultures. All came back negative. So they suspected it was a blood clot that formed and was causing the infection. So it was localized.
Had my follow up appointment early today. The skin between the two pieces of graft were like long blisters. Fluid filled. My surgeon figured out im allergic to the kind of stitches used to attach the graft to the skin. The weakest part between the two grafts. It’s why I’m posting pictures it’s easier to see than explain.
So im wondering if anyone else has had this blister reaction. My RFF phallo surgery was August 12. The thigh graft was like a little over a week later. Like my arm should be done with this crap, you know what i mean? Sigh….someone else has had to of gotten this kind of reaction or difficulty as well.
Oh ya, and i was instructed to only use lotion starting today. No more bacitracin and xeroform. And compression when leaving the house. But i know from experience that the blisters like to pop and ooze on my sheets or pillow while i sleep. So im thinking im going to wear that dang compression sleeve tonight! Unless someone tells me something like “oh no dude i did that blank happened! Give it air!” Id really appreciate some feed back.

There’s a spot on the middle of where my monsplasty incision is- I’m coming up on 3 weeks post op I have no fever, no chills or smell. My incision looks like it’s being chewed away and I’m concerned. I sent a photo to my care team and they replied with “put antibiotic ointment on twice a day”. I’m genuinely a bit concerned. This is gonna leave me with a nasty scar.

I'm 28 yo i had top surgery when I was around 15 years old. I had top surgery and they messed it up pretty bad (they didnt even change the size of my areolas ) ,i only take my shirt off in private and you can see the cave look when I wear shirts so I always wear baggy shirts or shirts with graphic designs where the chest is, also I know my body fat is high but back when I had my surgery I wasn't as big I was small and the results were worse as I think the extra body fat has filled in how bad the indents actually are. I don't know why they couldn't get it right or why they messed it up so bad however I never brought it up cause I figured I should be thankful they took them out BUT NO I shouldn't have been thankful as they done a horrible job and left me looking deformed. However as life went on I figured I'd do something about it and I'm booked in to see the surgeon next month to get my chest reconstructed again and im so excited/nervous as im not sure what kind of results ill get but she reassured me my chest will look a lot more normal than it looks now has anyone else gone through this with a chest that looked like mine and had it reconstructed again? I'd be greatful to see how it came out the 2nd time.

I am one month and one week post op keyhole surgery. I was told massages would help me twice a day, and maybe its just early, but I have been a little unhappy with one side of my chest.
I circled it in the last two images, but there are indents on both sides of my chest. The last photo is the side I am having the most trouble with. From the side, theres like a lump that makes my nipple tuck into itself (photo 2) and overall, there is no fluid, only hard tissue. Theres also indents on both side of my chest, which I circles. When massaging those indents, it honestly hurts and is sore (as is my chest generally).
I am genuinely wondering what to do, I am insecure about taking off my shirt and am wondering if I should be seeking some sort of revision .
In the first photo you can see where my pec at the top immediately just swoops down. I really hate this one side of my chest, and am sincerely wondering if this is just anxiety and its too early, or if I should seek my surgeon out to see what can be done.

Had surgery on 4/22, so I'm 4 weekss post-op and my incision opened a week ago. The nurse sent me some antibacterial ointment and just told me to keep it clean and dry.

Like, I keep seeing people here say physical stimulation feels very "nice," yet for me, it physically doesn't feel any different from, let's say, someone touching my arm. Physically and mentally. I'm sure someone shared an article here before where some transitioners with dementia were in huge distress because they forgot they transitioned and had SRS. I do wonder how much of that deformity is from almost ten years of body growth and weight gain, since many pooners gain and develop deformed breasts after top surgery. But this is still a great example of one of the many good reasons why one shouldn’t chop off a minor’s breasts.

@Caesar the SRS/Dementia scenario is so much more awful, when you realise the reason they will be reacting with such abject fear, is because of the state of the butchery itself.
