Insecurity and what I think is a bizarre form of acceptance that they are freaks. They couldn't just pick a sim of their desired gender and say they are trans or add an option at the hospital to simply switch between male/female model and make a sim actually trans. They had to add all the things that flag them as fake (scars, binders, wide shoulders). To me this is because even they know they will never be a real woman/man, so playing as a normal sim makes them uncomfortable and reminds them of that.
Something similar was in World of Warcraft, there was a Gender Change potion. It just literally changed you into the opposite gender. But in 2023 in Californias Blizzard offices, Gender is an offensive term. So now when you create a character in World of Warcraft, Male and Female are now Body 1 and Body 2. I don't play anymore so I don't even know if the potion still even exists because it might be offensive.
Probably why China cut the Blizzard contract, since they were going too far into LGBTQ+ land to be allowed for the Chinese public.
