It's because in its original form from what I observed was when transgender people acknowledged they couldn't change their biological sex and the threshold was gender dysphoria. The original point was that sex was for their biological sex but gender was their social identity so a MTF would put Sex: M Gender: F, which was why people were more open and accepting to it as it was acknowledging it wasn't changing their sex. In queensland in Oz years ago, there was loads of places that would take tourists out to snorkel on the great barrier reef etc, all the leaflets had this same big fish in pictures on them.
