I know nothing about the game, but I'd assume it either has a reference to troons (like Celeste) or it's a game that troons just latched onto (like Fallout New Vegas)

tumblr troons latched onto the game, and like to make the main characters + bosses into weird pseudopeople that have personalities that aren't at all relevant to the game

honestly I think most of them see the main character, Peppino, as some father figure they lacked who tries to accept their degeneracy and insanity, probably because the mtf trannies see themselves in his appearance (fat, short, and with body hair)
 
From time to time I think about uploading some of these memes to a trans thread somewhere and seeing what happens
