What is the ratio of 'MtF' vs 'FtM' trans?

I've seen claims of 4x to 2x more 'MtF' than 'FtM' trans to some places claiming they are almost equal. But in real life practically every trans I see or hear about is a dude. Outside of some very young girls being transed by their parents I can't think of a 'FtM' other than Ellen Page. Why is that?

Because self harm is a teenage girl thing. MtF trannies, on the other hand, are delusional and think they get an upgrade. The appeal of pooning has more specific scope.
In the uk and us it's also not true anymore, it's more pooners now who get medicalized. You think there's more trannies then poons because you also count middle aged dragqueens and other men in funny dresses or lady boys with silicone tits as trannies - but in the west, outside of thailand and places like iran, it's more girls than boys now who actually get butchered. There's some public NHS stats from Britain you can easily google. The problem with tranny stats is that only medicalized and sanctioned trannies are ever counted and there will always be more gay men who play dress up and claim they're women on the weekend in any society - but you can't prove that, scientifically.

off the top of my head i think the ratio is probably 20 : 1, just based on what i have seen. mtf troons seem to way out number pooners, but pooners do seem to be getting a bit more common as that social contagion takes hold. so id expect that ratio to drop slightly in the coming years.

Have you noticed it's really hard to get concrete numbers on any trans demographics?

Ever-changing and variable definitions, self-identification, patients "lost to follow-up," no coding/option at all for detransitioners.

That's how unscientific "facts" like the meme 41% become universally accepted. There's no real data.

If you asked people "born <male/female> and not cis," versus "have you taken legal, medical or social steps to be recognized as the opposite sex," the answers would be different, but you would still face difficulties getting people to answer honestly.

I think one way to get an estimate in the real world, without being defunded and preventing further research, would be if you polled every clinic in a city or region that offered HRT, and asked for numbers of estrogen scripts and numbers of testosterone scripts.

This leaves out the question of nonbinary entirely, and doesn't account for people who just change their screen name and start complaining, but there's no way to study the Enlightened Ones without being a bit of a shitlord.
