I don't think it looks that bad. Just avoid the incest route and ignore the cannonization? I've played games with incest before. I didn't go into it knowing there was one. But Girlish Love Revolution has an Onii-chan route. And no your character is not adopted. They say they're going to raise a cat together. I guess your imagination can do the rest. Personally I went for the terminally ill guy. He was best guy. Wild Arms 2 has implied incest. Don't tell me that the cripple wasn't banging his twin sister. I saw that cutscene.

Most of the time incest routes in VNs are with non-related characters. It takes balls to go the full monty with it. I don't really want to see it and would likely avoid that route. But as long as they are adults and the dev isn't trying to pair together minor siblings then it's not that outrageous. Just really gross and perverted. Dev's probably got some issues.
