Depends on the healthcare system, some of them have to go through a period of "living as their preferred gender", whatever the fuck that means, before they get the surgery, not sure if they have to do that for hormones too. "Living as their preferred gender" can mean anything from changing their name on a few accounts and having a utility bill for a few months in that name, or legally changing their name or asking everyone to call them that new name, but it's often taken to mean crossdressing.
