Even my political theory professor in college admitted that Butler was a terrible writer and that we should just try our best to read through Gender Trouble before class. Leaving aside the idea that he was talking to a senior admin and managed to completely convince them of his position in one conversation and presumably also convince them to start sucking his dick and give him their bank account information, why the fuck does he have the word dysphoria in scare quotes?

Think about it for just a second, the entire basis upon which trans healthcare can be considered a genuine medical necessity is the supposed psychological toll that weighs upon people with dysphoria, that's why trans activists talk so much about suicide rates, the idea that without getting hormones and surgery then people with dysphoria who feel a distressing mismatch between their bodies and their 'true' gender will end up taking their own lives. If its not about dysphoria, then its essentially just a very non-essential cosmetic procedure that shouldn't be funded publicly because its taking away time, people and money away from focus on much more pressing medical issues.

This idiot tells on himself constantly, he's never explained even within the framework of pro-trans healthcare how his absurdly anti-medicalist position makes any sense at all from a practical point of view, instead its informed consent and self ID all the way so any trender can tie up the medical system with their facile quest to look like a pretty lady (not even mentioning all the many negative outcomes that would prompt any responsible medical practitioner to dissuade people from pursuing such a self-destructive gambit for such arbitrary reasons), and ironically that will probably just make it harder for people with actual dysphoria, that is to say the group of people that its meant to make sense to medically transition in the first place for their mental health, to get the healthcare they want quickly and efficiently.

56 Dean Street's original purpose was an integrated service for gay men to deal with HIV - not only is it the largest HIV testing clinic in Europe, it also offers LGBT specific counselling to deal with a HIV diagnosis, along with helping people dealing with issues relating to chemsex and addiction (that often go hand in hand with risky sexual behaviour). They specifically go out of their way to deal with other high risk groups like female sex workers (they've got a special programme that lets prostitutes have walk ins with no appointments, not just gay men) and do sexual health outreach in places like illegal brothels, where the women might feel unable to access treatment. They offer rape counselling, prescribe PREP and PEP, make care plans and signpost to help with homelessness (with a specific focus on LGBT youth who may find themselves in precarious situations). They were one of the first places to diagnose AIDS in the world and it grew out of that. If you're a vulnerable gay man or prostitute in London who is struggling with anything, it's a safe place to go and they can crosslink you with any number of services. They've always been a force for good both in terms of reducing HIV/other STI transmission and specifically helping vulnerable groups in other ways.

I didn't realise that they did trans stuff but looking into it they offer a lot, which makes sense because in terms of HIV the category of MSM (males who have sex with males, so gay/bi men, "straight" men who fuck dudes and trans women) are at higher risk. Everything from gender identity counselling to safe hormone injections ( if you've been prescribed first) to liver function tests to make sure your spironolactone isn't fucking you up. That's a good thing, it's a one stop shop for everything trans beyond the initial diagnosis/prescription for 'mones. What the NHS is proposing is a reasonable solution; a trans specialist.

Tube's proposed alternative is to go into a regular hospital and speak to a regular endocrinologist who may well not have had specific training in trans medicine. You're going to get a cis doctor either way, because there's really not that many trans people in medicine. Then you're going to need a separate referral to speak to a plastic surgeon for the breast augmentation, and a separate referral for gamete storage, and a separate referral to a speech therapist, and a separate referral to one of the outsourced private clinics that does gender affirming surgery. All of whom will have long wait lists. It makes way more sense just to have a referral to a "tranny clinic" than to do all that, both in terms of saving taxpayer money on the NHS, in terms of reducing wait times for trans people and in terms of specialisation for trans specific issues (e.g. "I'm still getting blood on my dilator six months on, is this something to be concerned about?" - the GRS surgeon won't still be talking to you, your GP won't have a clue, and the proctologist/gynaecologist won't know and we don't have the resources to train all those people in it on top of everything else).

"Segregated" healthcare for specialties is the norm. You have to go to "segregated" healthcare for any range of conditions. If you find out you've got cancer, do you want your GP prescribing you chemo or do you want a consult with an oncologist? If you've got anything other than depression or anxiety, do you think you go to your GP or do you think you get referred to a "segregated" mental health unit? "Trans segregated healthcare" doesn't mean that if you've got a weird skin condition you have to go to a "trans segregated" dermatologist, unless maybe you're an FTM with chest acne from the T and there's someone who's especially good at dealing with that - and wouldn't you prefer that over a dermatologist who doesn't know what to do?

He really just doesn't care, he's latching onto the most unworkable and ignorant policies he encounters because it seems like the most radical thing he can support in a skin deep kind of way that improves his reputation as a great big trans activist, even though, as the previous post details, he really has no fucking clue at all about the intricacies behind this stuff but will gladly run his mouth off and denigrate and insult people with far more experience than him who've done infinitely more to help the groups he claims to be speaking for. He doesn't even bother to back up any of the stuff he says such as suggesting that trans people find it harder to get HRT or Gamete storage than cis people, or consider any legitimately good reasons why that might be the case such as people who aren't trans frequently having much more serious and pressing medical needs that necessitates them getting access to this kind of care much quicker than somebody who may not even have pressing dysphoria. Take Hysterectomies or Breast surgery for example, they don't just conduct these procedures willy-nilly on cis people to make sure that trans people have less of a window to get them and rub their hands in glee at the thought that trans people are unhappy, they prioritize people who have things like cancer, you know, conditions that will definitely kill them if left untreated.

I already talked about how utterly idiotic it is for him to poo-poo dysphoria diagnoses in this context considering that's the only way it can be reasonably argued that a trans persons needs these kinds of medical interventions for the sake of their health, mental health at least. Without that he's essentially arguing that trans people need to be privileged by the medical system in a way nobody else is so they can demand publicly funded healthcare, since he's in Britain, to give them unnecessary cosmetic procedures that will certainly happen at the expense of others. He doesn't even want to do it in a vaguely efficient manner as @AssignedEva points out. Its all just a big fucking facade so he can jerk himself off.

And on top of that he wants gender counselling to be led by trans people, lmao, I'm sure that certainly won't lead to a situation where the councilor in question has a huge vested interest in pushing patients who may not really know whats best for themselves down the path of intense transition with the massive interference on the body's natural progression that will entail. Lord knows how it will turn out if those people, often kids, realize this was the wrong path for them but they've done irreversible damage at that point.

A part of me almost wants to see these extreme measures TRA's like Oliver Thorn bang on and on about actually get implemented just because I know it will be an absolute disaster that will completely discredit the aggressive trans activist movement and show up how toxic and idiotic their ideology really is, so they can't just retreat into their victim status anymore. But then, it would probably mean ruining the lives of tons of young people and children, so its probably best to remain a hypothetical.

Best theory I’ve got is that he typed the letter in English, plugged it into Google Translate, and wrote out the results by hand. The machine would recognize “Ada” and “she” as feminine but default to masculine for “I,” as it would have no way of determining the speaker’s gender.
