I think with some people working out can take on neurotic overtones, and that in this case he just transferred that into trooning out. There's working out to get strong/stay healthy/look good, and then there's obsessively trying to customize yourself and getting way too into that. I think that for people like him trooning out is just a socially acceptable way for them to take their 'self-editing' fetish way beyond what's within the normal range. It's a weird combination of standard vanity and body dysmorphia with rapidly shifting goalposts. Kind of chilling in a way.
I was going to mention this - his reaction is so performative and so carefully designed to manipulate people (and especially his poor wife) into feeling sorry for him and reassuring him that he is loved and valued and did nothing wrong ever, while taking the attention and assurance away from the actual wronged party (his wife).
I agree with your concerns. If you call it in, I would try just suggesting a wellness check and say that he has been posting some alarming things on his social media and may be having a psychotic episode and be of risk to himself or others, or something similar. I think wellness checks are fairly usual in most Western countries and the authorities wouldn't hesitate too much to conduct one.
I don’t think I’ve ever heard of a wellness check here in Bongland. The authorities are very hands off with lunatics and don’t like to do anything until it’s too late, so I suspect any reporting of this guy will get you a “why are you so interested in him?” at best. He’s not committed a crime (that we know of), so the police can’t do anything anyway. At worst? The guy goes off in one when he realises someone was concerned about his weirdposting.
Huh, google tells me that the UK does have wellness checks by the police, but maybe that's more of a policy versus practice thing? I sort of assumed that if the UK police have got time to arrest young mothers for speaking wrongthink on Twitter, they must be willing to check up on psychotics and people having mental breakdowns and stuff as well. You're right that it might be risky, though, especially if the police don't do anything but say "hi someone online reported your twitter" or something.
Honestly, the more I read from FtM fujoshis, the more I'm convinced that it's not that women don't have paraphilias in general, it's just that the baseline is based on men and females with paraphilias just work differently . It's like how 50 Shades of Grey and other female-oriented smut is more about taking the emotional baggage of a psychopath rather than fairly lame BDSM. A lot of fujoshis seem obsessed with the emotional toll of being gay, rather than actually being gay.

Breast reductions are a different surgical technique though. These days breast reductions usually go around the nipple and never have to remove it. I mentioned this here before, but I know someone who got a non-FtM mastectomy because she had a strong genetic risk for breast cancer, and she also elected not to keep her nipples because the surgeon told her (in nicer words) that they might rot off and she wouldn't have sensation in them.
