They flay the arm and the leg because a deep graft (e.g. down to the muscle) is taken from the arm to make the sausage, while a shallow graft (e.g. just the surface layer of skin) is taken from the leg to cover up the arm graft.

Coke can penises usually arise from cases where the thigh itself is used to construct the sausage. Thigh skin is thicker than arm skin, especially if you need to double-roll it for a fake urethra.
