Meigh could very well be pregnant but be having a low birthweight baby, and this would make the bump a lot smaller. Last year I had this friend who was using opioids during pregnancy. Frequently people would remark about her tiny bump even when 7 months. Given Ethan’s proclivities and apparent access to street drugs it could be a possibility he’s ok not only ok with this, but also gives a reason that Meigh would stick with him if he became her source for opioids.

I have no evidence of this. just throwing it out there that she could be having a low birthweight baby and some follow up conjectures.
