Sir, As GPs we were delighted to see your report (“Javid inquiry into gender treatment for children”, news, Apr 23) outlining the approach being taken by the UK government in addressing the harm being done to under 18-year-olds who present to healthcare services with gender dysphoria. Ideology must never take precedence over the basic medical ethical principle of “Do no harm”. We are convinced that harm is being done to these young people. Litigation may follow against gender clinics as well as against the GPs who refer them there instead of addressing the underlying causes.

We believe that truly informed consent for the permanent, life- altering treatments that arise almost inevitably after the use of puberty blockers cannot be given by a minor. However, as GPs, are we nevertheless obliged to affirm, refer and then prescribe puberty blockers? What happens if we don’t?

We sought guidance from both NHS Scotland and from our own health board, NHS Western Isles. Neither was willing to provide us with clarity.
