Earlier in the stream he said Portugal has such a great healthcare system that they never even sent him a bill.

That seems odd. Just looking over the Wiki for Portugal's National Health Service, even citizens have to pay fees when using their socialized medical services at the end of their treatment, even if the treatment itself is covered. Unless their never sending him a bill means he fled the country before they could send him one.
