I think Jim is still laughing a little bit given the way he ended the tweet but I think it's mostly from the ridiculousness of the situation. Most drunks skip this step in the Alcohol related death pipeline because it is at this stage they get scared and they actually put the bottle down (to some extent) and get treatment to control those conditions. Ralph just won't. His fatty liver must be making it worse plus his habit of drinking hard liquor daily doesn't help either.
