She's specifically talking about people who feel obligated to shoulder all of the responsibility for their loved ones' mental well-being, and wear themselves out trying to singlehandedly "fix" their friend's depression, anger issues, or whatever, and think that if they can't spend all their emotional (or sometimes material) resources on somebody, then they're a bad friend and a bad person. Yes, I actually agree with Lily here. She's not telling you to abandon your friends if they're in a rough spot emotionally (at least in this paragraph she isn't), she's telling you not to blame yourself if you can't make people "better" on your own.

I personally disagree on the notion that facet of Mikaila is something she developed after dating Liliana. I think Mikaila just in general likes, or at least very used to be, the peacekeeper and a people's pleaser, putting everyone else she cares about before herself.

Liliana might be a emotionally manipulative abuser but she can't change someone's personality, not after just a few years of dating at least. She just happened to find someone that already had this trait and has exploited her for all of it's worth since then, getting all the free art and labor she wants. She'll keep milking Mikaila until Mikaila herself snaps out of it or there's nothing else to milk, but she won't notice by herself that something is wrong as long she can get what she wants.
