As a previous poster says it depends on why it’s dying. If it’s an infection then the guy needs major IV antibiotics and hope that it clears it, then you still need to see what happens with the dead flesh. If it’s loss of blood flow, it’s a bit like frostbite injury - you wait to see what cleans up and falls off, tidy it up and clip away what’s dead but attached (I recommend ranulph fienns book where he does this himself in his garden shed with a frost bitten toe or finger after an expedition, when men were men, etc…)
The main question is how deep it goes. It looks deep to me but it’s impossible to say. Dead flesh will rot and give you bacterial gangrene after a while if it’s not removed anyway. So anything in the outside will just drop off/be debrided. The internal stuff is more of a worry. He has a catheter going into the urethra and there’s necrosis ‘near’ it. You risk doing a LOT of damage taking the catheter out but also by leaving it in.
Worst case scenario is death by infection due to a bit left in there. Most probable is damage to urethra, loss of significant flesh where the black stuff is and some nasty scarring and functional loss. It’s a very nasty pic but if you search the site for fourniers gangrene (really, gore, be warned) you’ll see one possible outcome

I don't agree that all troons (FtM and MtF) are mentally ill as gets suggested in here frequently. They are living on pure hope, fetishistic impulses and a desire to belong to something. All of that leads to some terrible decisions, but I wouldn't consider them all mentally ill. Dr Keelee Macphee in NC gave this guy TopNeppers an amhole about 54 days ago and he documented all the nasty shit that went on in it. There was necrosis, granulation, dehiscence, and what appears to be like polyps on his stinkditch.
The photos are really foul and gruesome in the early ones as you can see his amhole partially rotted away to leave portions of his fake labia missing. If anything, it looks closer to a human ear profile than anything resembling a vulva.
