I think it's because women have a higher and different body fat distribution than men. Having no body fat removes the feminine features that should be present.
I don't know much about Eugenia, but doesn't she live with her mom? Why doesn't her mom have an intervention?

Her Mom doesn't care. She's the one who takes the pictures and videos whenever Eugenia is outside. You can see her reflection in some pictures. When Eugenia had a 5150 her mother said she'd have a heart attack if she didn't come back.
Her mother is a huge part of the problem.

I think the mother 'likes' having a skinny sick daughter. Maybe letting her slowly starve to death is Eugenia's punishment for some sin imagined or otherwise the mother can't forgive, because why frighten the daughter into leaving a program that would save her life?

She's been in multiple videos. Giant fat ass is a bit of an overstatement. Mother of multiple children who let herself go on probably more than one medication who's side effect is weight gain would be my description. She also doesn't strike me as the type to turn down a complimentary bottle of wine, mimosas,etc. The booze adds up, especially after menopause.
Personal opinion because PL and TMI of which I won't go into because nobody wants that shit: Eugenia is her drug of choice and it goes deeper than weight. Codependency is a heavy yolk.

Usually this tends to involve some kind of medication adjustment, exercise, and keeping within a calorie limit. With their wealth, it's easy to find a trainer and a doctor who will make the process less difficult. Maybe even hiring a dietary chef to prepare and freeze a week's worth of tasty diet food every week to avoid temptation.
Because if the mother truly has a bad heart, being and remaining this heavy will likely kill her in a few years. Weight puts a massive strain on on weak hearts, and she's being extremely stupid by letting herself remain like she is.

After seeing how utterly distraught she was over the diaper allegations, it seems likely she might be deliberately flashing to disprove the people who are saying she's incontinent. It's really sad to see, especially when you know there's people getting off to it.
