Only Ralph does pathetic shit like that because he’s pathetic, dumb, little man with a giant inferiority complex

Mental deterioration is basically "gradually, then all at once" especially when combined with substance abuse. My father was an addict who loved alcohol and coke and you could see his mental state falling apart until he hit that point where it's like a switch flips and he just completely dropped off and he was gone. Maybe you get moments of lucidity but for the most part once the path of deterioration gets far enough you never come back from it. Even people who get clean can't really reverse the damage, they just become more functional and with a better locus of control over their lives. If Ralph suddenly got clean tomorrow and was against the odds able to stick to it, at best he would be able to just better manage his emotions and public freakouts. Maybe, very maybe, some of his mental processes would slightly recover. It's pretty obvious however his impulse control is degrading.
