All in all, given Nick's priorities, his (and Kayla's) documented behavior, and most importantly Nick's strong reaction to whatever it was in June/July, the evidence points toward something marital unless Nick has another hidden value other than achieving orgasm. It's the only thing Nick is so sensitive about that he would delete any threat to it. Even though Nick has embraced more of his degeneracy, he will still try to defend his marriage.
It doesn't have to be sex with someone else (at least not at that time) but given how they try to "be adventurous" in order to keep the marriage alive (and Nick does recommend this to others), it wouldn't be surprising if one of them went too far and "someone" got hurt.
