The thing I'm wondering was if it was a medically necessary surgery why wouldn't Nick just say what it was? He already shared the already very personal information about how long they couldn't have sex.

If it was some sort of cosmetic surgery, this would fit the pattern where Nick overshares about something before coming to a screeching halt where he doesn't want to tell the whole story.
