If what's you're saying is accurate and tax payers paid for his SRS, it's worse than in the EU with "free" socialized healthcare, because some stuff is not covered by that, and some stuff you would have to go through psychiatry, endocrinology and get all sorts of "seals of approval" from various medical specialties before you even start HRT, and for SRS you can wait yet another 5-10 yrs or w/e, in fact it's so rare that people just don't even try it locally and instead go to Thailand or some other similar place and pay it in full.
 
Kevin lives in Colorado, which has state laws requiring insurance companies cover all trans surgeries. All of this, plus his upcoming breast augmentation, is covered by Colorado Medicaid. You’ll often see Kevin bring up Kaiser, which is an option with CO Medicaid.

As mentioned he's on Colorado Medicaid. It's partially funded by the federal government and partially funded by the state government (the specific split differs by state and I'm not going to look this up, it's just fair to say American taxes pay for it), they outsource its management to an insurance company (Kaiser, but possibly multiple ones, again depends on the state) but everything about it is state determined* except for the day-to-day functioning which Kaiser handles. Essentially the government pays Kaiser to throw Kevin (and the other Medicaid cases) into its pools with every private pay customer, there are limitations and specifics which is why certain doctors opt out of Medicaid completely but a hospital would not do this (in many cases it cannot do this) and Kevin's main effort would be finding doctors/surgeons/etc. that would take the Medicaid payment which is often lower than private insurance because it's a monopsony price.

Kevin's elective and cosmetic surgeries plus HRT cult injections are considered to be "medically essential" because he finagled a gender dysphoria diagnosis. (That troons are fighting to eliminate this is one of the funniest troon measures since it would kill this.) In some cases these "medically essential" cosmetic elective surgeries got prioritized during the pandemic because they were free money with minimal risks, which is why there were stories of women patients getting shunted aside so troons could have the operating rooms. Assume yourself as a surgeon who can perform every procedure, a woman comes to you for something complicated that she might die from (maybe no matter what) and there's justifiable reasons to pushing it off while a troon comes to you for a quick dick chop that will put money in your pocket today. You can see the dilemma you face, especially if you're a dick chopper for hire really enjoying that there's simply no standards in the field nor any way to measure the results since the troons have all adopted a delusion field that says every single example is specifically someone else's problem they will be exempt from.

So there has been conflicting information from different parties. For all I know it wound up being approved after all, but he was informed by his insurance that it wasn't at one point in Humbert Humbert's pre-op post. Again, health insurance has a storied history of refusing payment even for undeniably legitimate, necessary, unavoidable care. I don't believe a state law saying they cover SRS will undo that.

Given there were conflicting reports of denial/approval from various parties in the process, I would not be surprised to hear that the process takes a little while before funds are refused and the hospital tells the patient that they are on the hook. It works. My hospital gave me an overall discount of 40 percent for paying cash. They can do it because by charging straight cash the hospital cuts out the insurance company completely. They love not having to battle them on a line item bill.
