His head looks too small for his body. I have zero clue about medical shit, but did puberty blockers mean his head head stayed smaller than if he’d been allowed to develop normally (like his dick head did)? Or is his body just so fat from eating to stuff down despair that it makes his head look smaller?

Except for rare cases, people gain weight uniformly. You gain weight? Your face and cheeks are fuller. His face seems "normal" compared to his body, which is also a side effect of the so many hormones he's taking. The food just gives him a few fat rolls here and there, but the hormones are inflating him like a balloon.

Don't forget, if he was on puberty blockers then most likely a lot of him is under developed integrally. His metabolism could be completely destroyed, his immune system ruined, and his heart and lungs aren't fully grown. Brittle bones too that continue to deteriorate.
