Going to disagree here. XR doesn't work well for everyone. Also, if you are on and taking 40+ mg (I went back to see where we got 40 mg as his dose, and didn't find it, but I'll assume you did and go with it), you are having to take multiple pills/day (afaik, they are manufactured in increments up to 30 mg, so you'd have to take > 1 to get to 40+ mg; xr also maxes at 30 mg/pill, I believe).
For reasonable people who may be on a high dose, having multiple pills means you may then opt to take less than the full prescribed amount on any given day, if you don't want or don't need the second or third. Being able to do that means you can better assess whether you need that much daily, in general and specific, and potentially lower the prescribed dose, which also mitigates against tolerance and dependency.
That said, there is no way this chump walked out of an initial appointment with a 40 mg rx. The max rx (without jumping through coverage hoops) is 60 mg/day. Initial prescriptions start low (5-15 mg I'd guess as the typical) and may increase if ineffective. Blasting someone who may not need much with 40 mg is reckless and pointless.
