I think you're just looking at the wrong level of government. The city councils in Boston, New York, etc. all pass the same kind of stuff that the western cities do. I think if there's a major difference it's that the Eastern areas have more established interests that are unwilling to allow them to experiment with utopian schemes. I think because of the more rooted nature of the populations there's more inherent opposition in the East if someone proposes something like "we should tear down all the buildings of Wall Street to have an open air rehabilitation clinic for those suffering social injustice." There will be a bunch of unions and banks and long standing progressive institutions like the New York Times to go NIMBY. The centrality of the media probably also plays a factor as stories about random shit in NYC and DC and even Boston somehow become national news stories in a very different way than anywhere else, it's not surprising that most people believe half the country lives in those locations, and so you tend to find out more only after when the disaster is occurring rather than the earlier controversy when the idea was proposed and pushed through.
I was going to say that I think the Western states may be more polarized in that the Democrats are more progressive and the Republicans more conservative, but Massachusetts and New York are quite like that too, the occasional moderate Republican who wins (see also: Hogan) rarely carries over to the rest of the party and is usually more of a reaction against Democratic excesses than an endorsement of the Republicans as a whole.
One understated factor to this? Blacks. Seattle is 7% Black, Portland is 6% Black, San Francisco is 5% Black and Los Angeles is 8% Black. Blacks are moderates in the Democratic coalition as we saw in 2020 when they swung behind Biden as everyone else was running to the left after Bernie. Black organizations may be just as far-left but Black voters aren't. (edit: As we saw when Tony threatened to drive most of the Blacks out of the Maryland Democratic Party.)

Self-described weight, weight management intentions, unhealthy weight management, prescription of psychiatric medications, and medications associated with weight gain were associated with BMI category. Use of psychiatric medications (54.8%) and medications associated with weight gain (39.5%) was associated with BMI in the overweight/obese categories. Youth with obesity most often reported unhealthy weight management. In CART models, self-described weight was the strongest predictor of BMI category.
Conclusion: TGD youth have high rates of underweight and overweight/obesity. Unhealthy BMI should be treated as part of gender-affirming care. Self-described body weight is associated with weight category.
